Irinotecan Hydrochloride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cascara Sagrada interacts with.

Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Here's the gist for irinotecan (Onivyde): your liver uses an enzyme called CYP3A4 to help process this chemo drug. Lab studies hint that cascara sagrada might gently speed that enzyme up. In theory, a faster enzyme could clear the drug a little quicker, which might slightly weaken how well your treatment works. I want to stress this is only test-tube evidence, the effect was much weaker than known enzyme boosters, and we don't have real human data showing it actually happens.

Still, irinotecan is serious cancer medicine, so I wouldn't take chances. Please don't start cascara without first checking with your oncology team or pharmacist. They can suggest a gentler, better-studied way to handle constipation during chemo.

  • Irinotecan lipid complex is used in combination with other medications to treat pancreatic cancer that has spread to other parts of the body that has worsened after treatment with other chemotherapy medications.
