Mometasone, formoterol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cascara Sagrada interacts with.

Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Your Zenhale inhaler combines two medicines: mometasone (an inhaled steroid that calms airway swelling) and formoterol (a long-acting opener that relaxes your breathing tubes). Both are partly processed by a liver enzyme called CYP3A4. The theory here is that cascara sagrada might gently speed that enzyme up, which could clear these medicines a little faster and, in theory, make them work a touch less well over time. You'd notice this as less control of your asthma symptoms, not as new side effects.

Honestly, I wouldn't lose sleep over this one. The evidence is only from lab studies, the effect looks weak, and inhaled doses are small to begin with. Still, if you're considering cascara, run it by your pharmacist or doctor first, since it has its own cramping and electrolyte concerns.
