Pitolisant Hydrochloride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cascara Sagrada interacts with.

Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Here's the gist for Wakix (pitolisant): your liver uses an enzyme called CYP3A4 to help clear it. Lab studies hint that cascara sagrada may speed that enzyme up a little, which could move pitolisant out of your body slightly faster. If that happened, the medicine might not work quite as well, so you could notice more daytime sleepiness or your cataplexy episodes creeping back.

That said, this is only a theoretical concern based on test-tube research, and the effect looked weak. Occasional, short-term use of cascara for constipation probably won't make a real difference. Still, since you're relying on pitolisant to stay awake and safe, it's worth a quick chat with your pharmacist or doctor before adding it.

Cascara sagrada is a stimulant laxative made from the aged bark of a Pacific Northwest tree, used mainly for short-term relief of constipation. Because it can cause cramping, dehydration, and electrolyte problems—and was removed from over-the-counter laxative products in the U.S. due to safety concerns—it should only be used briefly, if at all, and with guidance from a pharmacist or doctor.

What Is Pitolisant Used For?

Brand name: Wakix

Wakix (pitolisant) is used to treat two hallmark problems of narcolepsy: excessive daytime sleepiness (feeling overwhelmingly tired and unable to stay awake during the day) and cataplexy (sudden episodes of muscle weakness or loss of muscle control, often triggered by emotion). It is approved for adults and children 6 years of age and older.

Both uses are specific to Wakix tablets taken by mouth. The drug is not approved for sleepiness or cataplexy caused by conditions other than narcolepsy.
