Quetiapine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cascara Sagrada interacts with.

Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Your body relies on a liver enzyme called CYP3A4 to clear quetiapine out of your system. Lab studies hint that cascara sagrada may gently speed up that enzyme, which in theory could break quetiapine down a little faster. If that happened, you might get slightly less benefit from your dose, meaning the medicine that helps steady your mood, thinking, or sleep might not work quite as well. Quetiapine is an active drug, not a prodrug, so this points toward weaker effect, not stronger.

Honestly though, this is a theoretical concern based only on test-tube research, and the effect was small and far weaker than known enzyme boosters. I wouldn't lose sleep over occasional short-term use, but please check with your pharmacist or doctor before pairing them, since you shouldn't change your quetiapine on your own.

Cascara sagrada is a stimulant laxative made from the aged bark of a Pacific Northwest tree, used mainly for short-term relief of constipation. Because it can cause cramping, dehydration, and electrolyte problems—and was removed from over-the-counter laxative products in the U.S. due to safety concerns—it should only be used briefly, if at all, and with guidance from a pharmacist or doctor.

What Is Quetiapine Used For?

Brand names: Seroquel, Dopaquel

Quetiapine treats schizophrenia and bipolar disorder in adults and certain age groups of children and adolescents. For bipolar disorder, it is approved for manic episodes (both as a stand-alone treatment and alongside lithium or divalproex) and for depressive episodes.

The extended-release form of quetiapine is also approved as add-on therapy for major depressive disorder (MDD) in adults who are already taking an antidepressant. The regular tablet form is not approved for this use.
