Selinexor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cascara Sagrada interacts with.

Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Here's the gist for Selinexor (Xpovio). Your body partly clears this medicine using a liver enzyme called CYP3A4. Lab studies hint that cascara sagrada may speed that enzyme up a little (much more weakly than a strong inducer like rifampin). If that happened in real life, your body could clear selinexor a touch faster, which in theory might make it slightly less effective against your myeloma or lymphoma. Selinexor is not a prodrug, so faster clearance means less drug effect, not more.

The honest part: this is only test-tube evidence and the effect looks small, so I wouldn't lose sleep over it. Still, selinexor is a serious cancer drug, so please check with your oncologist or pharmacist before using cascara sagrada.

Cascara sagrada is a stimulant laxative made from the aged bark of a Pacific Northwest tree, used mainly for short-term relief of constipation. Because it can cause cramping, dehydration, and electrolyte problems—and was removed from over-the-counter laxative products in the U.S. due to safety concerns—it should only be used briefly, if at all, and with guidance from a pharmacist or doctor.

What Is Selinexor Used For?

Brand name: Xpovio

Xpovio (selinexor) is used to treat multiple myeloma in adults. When used together with bortezomib and dexamethasone, it is for adults who have had at least one prior treatment. This combination is sometimes called XVd.

Xpovio is also used with dexamethasone alone (called Xd) for adults with relapsed or refractory multiple myeloma — meaning the cancer came back or stopped responding — who have already tried at least four prior therapies and whose disease no longer responds to several other standard drug types.
