Theoretically, use of cowhage might decrease the clinical effects of antipsychotic drugs.

The medication you're asking about is a combo pill: amitriptyline (an antidepressant) plus perphenazine, which is an antipsychotic. The perphenazine half works partly by quieting dopamine activity in the brain. Cowhage is a natural source of L-dopa, the raw material your body turns into dopamine. So in theory, cowhage could push dopamine up while perphenazine is trying to keep it in check, working against each other.

If that tug-of-war happened, the perphenazine might not control symptoms as well. I'll be honest, this is a theoretical concern based on how these compounds behave, not on real patient reports, so I don't want to alarm you. Still, because cowhage can be potent, please run it past your pharmacist or doctor before combining them.

Amitriptyline, Perphenazine also falls under TRICYCLIC ANTIDEPRESSANTS (TCAs).

Theoretically, use of TCAs might reduce the levels and clinical effects of cowhage.
Cowhage contains levodopa. Use of TCAs might reduce the absorption of levodopa. Some case reports describe patients that developed hypertension and dyskinesia when taking both levodopa and TCAs.

Triavil/Etrafon combines amitriptyline (a tricyclic antidepressant) with perphenazine (an antipsychotic). Cowhage carries natural L-dopa, the dopamine building block used in Parkinson's care. On paper, this combo pulls in two directions. The amitriptyline side may slow your gut and reduce how well L-dopa from cowhage gets absorbed, so the supplement could work less than expected. A handful of older case reports also describe people on both who saw spikes in blood pressure and unwanted muscle movements (dyskinesia).

There's an extra wrinkle: perphenazine actually blocks dopamine, working against what cowhage tries to do. This is theoretical, based mostly on how these drugs behave rather than solid studies. Still, please check with your pharmacist or doctor before mixing them, and don't change your medicine on your own.
