Fever Bark and Aripiprazole Lauroxil

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Why this applies to Aripiprazole Lauroxil

Aripiprazole Lauroxil falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Fever Bark interacts with.

Theoretically, fever bark might increase the levels and clinical effects of CYP2D6 substrates.
In vitro research suggests that yohimbine, a constituent of fever bark, inhibits CYP2D6 enzyme activity. The effects of fever bark itself are unclear.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for your Aristada injection (aripiprazole lauroxil). Your body relies on a liver enzyme called CYP2D6 to help clear this medication. Fever bark contains a yohimbine-like compound that, in lab dishes, appears to slow that same enzyme down. If that happened in real life, aripiprazole could build up a bit and its effects could feel stronger, things like drowsiness, restlessness, dizziness, muscle stiffness, or tremor.

I want to be honest about the strength here: this is only a theoretical concern based on test-tube work, and how fever bark behaves in actual people isn't clear. Still, since this is a long-acting injection you can't easily undo, and fever bark itself can affect blood pressure and heart rate, I'd skip it and check with your doctor or pharmacist first.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).

From our pharmacist’s overview on the Aripiprazole Lauroxil page

“Aripiprazole lauroxil (Aristada) is a long-acting antipsychotic injection, and because each dose keeps working for weeks, a bad supplement pairing cannot be undone by skipping a pill. So the 16 major-rated entries among its 233 interactions on file deserve to be understood first.”

About Fever Bark

What is Fever Bark?

Fever Bark
  • Fever
  • Diarrhea
  • Malaria (traditional)
  • Digestive complaints
  • Bitter tonic

Fever bark is the bark of the Australian tree Alstonia constricta, used in traditional medicine for fever, diarrhea, and infections. There is very little reliable human research to support these uses, and the bark contains potent alkaloids that may cause serious side effects. It is best avoided unless used under direct medical supervision.
