Theoretically, fever bark might increase or decrease the effects of stimulant drugs.
Reserpine, a constituent of fever bark, might increase or decrease effects of sympathomimetics. Also, yohimbine, another constituent of fever bark, has sympathomimetic effects and increases blood pressure in a dose-dependent manner. It is not known if this would occur with fever bark.

Here's the piece that matters for Fiorinal: the part of this combo that interacts with fever bark is the caffeine, which is a stimulant. Fever bark contains plant compounds that can pull your body in opposite directions. One (a reserpine-like alkaloid) tends to calm the nervous system, while another (a yohimbine-like alkaloid) can nudge blood pressure and heart rate up. So in theory, combining them with caffeine could either blunt or boost caffeine's stimulating effects.

Honestly, this is mostly theoretical and based on weak, anecdotal evidence, so I wouldn't lose sleep over it. Still, if you noticed jitteriness, a racing heart, or unusual blood pressure swings, that's worth mentioning. Fever bark is potent and best avoided unless your doctor is supervising. Please check with your pharmacist or doctor before mixing the two.

Theoretically, combining fever bark with antiplatelet or anticoagulant drugs might have additive effects; however, this has not been reported in clinical research.
Research in healthy adults shows that taking yohimbine, a constituent of fever bark, in doses of 8 mg or more, seems to inhibit platelet aggregation in vitro by binding to the alpha-2 adrenoceptor. The effects of fever bark itself are unclear.

The piece of your Fiorinal to focus on here is the aspirin. Aspirin already thins the blood a bit by making platelets less sticky. Fever bark contains a yohimbine-like compound that, in lab dishes, also nudged platelets toward being less likely to clump. So in theory, pairing the two could add up and make you bruise or bleed a little more easily, or take longer to stop bleeding from a small cut.

I want to be honest about the evidence though: this is only test-tube and animal data, and it has never been shown in real people. The effect of fever bark itself is genuinely unclear. I'd lean away from fever bark anyway because its alkaloids can mess with blood pressure and heart rate, but please run it by your pharmacist or doctor before mixing the two.

Fever bark is the bark of the Australian tree Alstonia constricta, used in traditional medicine for fever, diarrhea, and infections. There is very little reliable human research to support these uses, and the bark contains potent alkaloids that may cause serious side effects. It is best avoided unless used under direct medical supervision.

What Is Aspirin, Butalbital, and Caffeine Used For?

Brand name: Fiorinal

Butalbital, aspirin, and caffeine capsules are used to relieve the symptom complex of tension headache — also called muscle contraction headache. This includes headache pain, psychic tension, and muscle tightness in the head, neck, and shoulders.

This medicine is intended for short-term use. Evidence supporting its safety and effectiveness for repeated or long-term use in recurrent headaches is not available, and extended use is not recommended because butalbital can be habit-forming.
