Aspirin, Caffeine, Codeine Phosphate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Fever Bark interacts with.

Theoretically, fever bark might increase the levels and clinical effects of CYP2D6 substrates.
In vitro research suggests that yohimbine, a constituent of fever bark, inhibits CYP2D6 enzyme activity. The effects of fever bark itself are unclear.

Here's the wrinkle with this combo product: the codeine piece is a prodrug. Your body has to convert codeine into morphine using a liver enzyme called CYP2D6 before you get real pain relief. Fever bark contains yohimbine-like compounds that, in lab studies, can slow that same enzyme down.

So if fever bark blocks CYP2D6, you'd actually make less morphine from the codeine, meaning weaker pain relief, not stronger. The aspirin and caffeine parts aren't really affected here. Keep in mind this is only theoretical, based on test-tube data, and the real effect of fever bark in people is unknown. Honestly, fever bark carries its own risks regardless. Please check with your pharmacist or doctor before mixing the two.

Reserpine, a constituent of fever bark, might increase or decrease effects of sympathomimetics. Also, yohimbine, another constituent of fever bark, has sympathomimetic effects and increases blood pressure in a dose-dependent manner. It is not known if this would occur with fever bark.

Here the piece that matters most is the caffeine in this combination product. Fever bark contains plant alkaloids, including a yohimbine-like compound that can act as a stimulant and nudge blood pressure and heart rate upward. Stacked on top of caffeine, that could theoretically add up to feeling jittery, a racing or pounding heartbeat, trouble sleeping, or higher blood pressure. Fever bark also contains a reserpine-like compound that can pull the other direction, so honestly the net effect is unpredictable.

I want to be straight with you: this is based on anecdotal, theoretical reasoning, not solid human studies. Fever bark itself is best avoided anyway because of its potent alkaloids. Please check with your pharmacist or doctor before pairing them, especially if you have heart or blood pressure concerns.

Theoretically, combining fever bark with antiplatelet or anticoagulant drugs might have additive effects; however, this has not been reported in clinical research.
Research in healthy adults shows that taking yohimbine, a constituent of fever bark, in doses of 8 mg or more, seems to inhibit platelet aggregation in vitro by binding to the alpha-2 adrenoceptor. The effects of fever bark itself are unclear.

This combination pill already has aspirin in it, which thins your blood a bit by making platelets less sticky. Fever bark contains a yohimbine-like compound that, in lab studies, also seemed to make platelets clump less. So on paper, stacking the two could add up and slightly raise your bleeding risk. In real life that might look like easier bruising, nosebleeds, or bleeding gums.

I want to be honest about how shaky this is. The concern comes only from test-tube work, not from people actually taking them together, so it is theoretical and rated minor. Fever bark's alkaloids can also mess with blood pressure and heart rate, which is a bigger reason to be cautious. Please check with your pharmacist or doctor before pairing them.

Fever bark is the bark of the Australian tree Alstonia constricta, used in traditional medicine for fever, diarrhea, and infections. There is very little reliable human research to support these uses, and the bark contains potent alkaloids that may cause serious side effects. It is best avoided unless used under direct medical supervision.
