Clomipramine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Fever Bark interacts with.

Theoretically, fever bark might increase the levels and clinical effects of CYP2D6 substrates.
In vitro research suggests that yohimbine, a constituent of fever bark, inhibits CYP2D6 enzyme activity. The effects of fever bark itself are unclear.

Here's the gist with Clomipramine (Anafranil). Your body relies on a liver enzyme called CYP2D6 to break this medication down. Fever bark contains a compound similar to yohimbine that, at least in lab studies, may slow that enzyme. If it does, clomipramine could clear out more slowly and build up in your system.

Higher levels could mean stronger side effects like a dry mouth, constipation, drowsiness, dizziness, a racing heartbeat, or trouble urinating. I'll be straight with you though: this is theoretical and based only on test-tube data, so we really don't know how fever bark behaves in real people. Fever bark also has its own heart and blood pressure effects, which is another reason to be cautious. Please check with your pharmacist or doctor before combining them.

Theoretically, CYP2D6 inhibitors might increase the levels and clinical effects of yohimbine, a constituent of fever bark.
In vitro and clinical research shows that the fever bark constituent, yohimbine, is metabolized by CYP2D6 isoenzymes.

Here's the thing to know with fever bark and clomipramine (Anafranil). Clomipramine isn't a prodrug, so it doesn't need an enzyme to switch it on. But it does act as a CYP2D6 blocker, and that same enzyme is what your body uses to clear the yohimbine-like alkaloids in fever bark. So the concern runs the other direction here: the medication may slow the breakdown of the bark's compounds, letting them build up.

In plain terms, you could see more of fever bark's stimulant-type effects, like a racing heart, jitteriness, anxiety, or a bump in blood pressure. This is theoretical and based on how the chemistry should behave, not on strong human studies. Still, fever bark carries potent alkaloids on its own, so I'd run it by your pharmacist or doctor before combining the two.

Fever bark is the bark of the Australian tree Alstonia constricta, used in traditional medicine for fever, diarrhea, and infections. There is very little reliable human research to support these uses, and the bark contains potent alkaloids that may cause serious side effects. Clomipramine — sold as Anafranil and under the generic name Clomipramine Hydrochloride — is approved to treat obsessions and compulsions in people with Obsessive-Compulsive Disorder (OCD). This includes adults and children/adolescents ages 10 to 17.

To qualify for treatment, the obsessions or compulsions must cause significant distress, take up a lot of time, or noticeably interfere with work, school, or social life.
