Dorzolamide, Timolol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Fever Bark interacts with.

Theoretically, fever bark might increase the levels and clinical effects of CYP2D6 substrates.
In vitro research suggests that yohimbine, a constituent of fever bark, inhibits CYP2D6 enzyme activity. The effects of fever bark itself are unclear.

Here's the gist for Cosopt eye drops. The timolol part is a beta blocker that your liver clears partly through an enzyme called CYP2D6. Fever bark contains a yohimbine-like compound that, in lab tests, can slow that same enzyme down. In theory, if more timolol gets absorbed from your eyes into your bloodstream and clears slower, you could see more of timolol's body-wide effects, like a slower heart rate, lower blood pressure, lightheadedness, or trouble breathing in folks with asthma or COPD.

I want to be honest though: this is built on test-tube data only, and whether fever bark itself does this in real people is unknown. Timolol is not a prodrug, so the worry is more effect, not less. Since fever bark already carries safety concerns, please run this combo by your pharmacist or doctor first.

Fever bark is the bark of the Australian tree Alstonia constricta, used in traditional medicine for fever, diarrhea, and infections. There is very little reliable human research to support these uses, and the bark contains potent alkaloids that may cause serious side effects. It is best avoided unless used under direct medical supervision.

What Is Dorzolamide and Timolol Ophthalmic Used For?

Brand name: Cosopt

Dorzolamide and timolol ophthalmic eye drops are used to reduce elevated intraocular pressure (pressure inside the eye) in people with open-angle glaucoma or ocular hypertension (higher-than-normal eye pressure without signs of glaucoma damage). It is specifically intended for patients who have not responded well enough to beta-blocker eye drops alone.

This combination is available as Cosopt, Cosopt PF, and Dorzolamide Hydrochloride and Timolol Maleate eye drops. Cosopt PF is the preservative-free formulation, which may be preferred for people sensitive to preservatives.
