Goji and Abrocitinib

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Goji interacts with whole categories of medication. Abrocitinib falls into 2 of those categories, so it can interact with Goji in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES

1 of 2

Why this applies to Abrocitinib

Abrocitinib falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Goji interacts with.

Theoretically, goji berry might inhibit CYP2C19 and reduce metabolism of CYP2C19 substrates.
In vitro research shows that goji berry tincture and juice inhibit CYP2C19 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP2C19 substrates. However, this has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here is the gist for your eczema medication, Cibinqo (abrocitinib). Your body uses a liver enzyme called CYP2C19 to help break this drug down and clear it. Lab studies suggest goji berry may slow that same enzyme. If that happened, abrocitinib could build up a bit higher than expected, which might mean more of its known side effects, like nausea, headache, acne, or a greater chance of infections since this drug calms the immune system.

The reassuring part: this is only seen in test-tube research, not in real people, so it is theoretical. Everyday goji berries in food are very unlikely to matter. If you are thinking about concentrated goji supplements, just run it by your pharmacist or doctor first so they can keep an eye on you.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

2 of 2

Abrocitinib also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Theoretically, goji berry might inhibit CYP2C9 and reduce metabolism of CYP2C9 substrates.
In vitro research shows that goji berry tincture and juice inhibit CYP2C9 enzymes. Additionally, multiple case reports suggest that goji berry concentrated tea and juice inhibit the metabolism of warfarin, a CYP2C9 substrate. Concomitant use with goji may decrease metabolism and increase levels of CYP2C9 substrates.

Here's the gist: your body uses an enzyme called CYP2C9 to help clear abrocitinib (Cibinqo). Goji berry may slow that enzyme down a bit, at least in lab studies and in a few case reports involving warfarin. If goji blocks that pathway, abrocitinib could clear more slowly and build up in your system, which can mean stronger effects and a higher chance of side effects like nausea, headache, acne, or, more importantly, increased infection risk since this drug calms the immune system.

That said, this is mostly theoretical and based on weak evidence, and it usually involves concentrated goji juice, tea, or supplements rather than a few berries in your trail mix. I wouldn't panic, but I would give your pharmacist or doctor a heads-up before starting goji supplements.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.

From our pharmacist’s overview on the Abrocitinib page

“Bleeding is where I'd focus first with abrocitinib (Cibinqo), an eczema pill that can itself lower platelet counts. Of the 290 supplements and herbs with an interaction on file, 202 have blood-thinning or antiplatelet activity, and two of the three major-rated entries, danshen and piracetam, sit in that group.”

About Abrocitinib

What Is Abrocitinib Used For?

Brand name: Cibinqo

Cibinqo (abrocitinib) is used to treat moderate-to-severe atopic dermatitis (eczema) in adults and children 12 years of age and older. It is specifically for people whose eczema has not been adequately controlled with other systemic (whole-body) treatments, including biologics, or when those treatments are not a safe option.

Cibinqo is not recommended to be used at the same time as other JAK inhibitors, biologic immunomodulators, or other immunosuppressants (medicines that reduce immune system activity).
