Esomeprazole Magnesium falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Goji interacts with.
In vitro research shows that goji berry tincture and juice inhibit CYP2C19 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP2C19 substrates. However, this has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Nexium (esomeprazole) and goji. Your body uses a liver enzyme called CYP2C19 to break down esomeprazole. In lab dish studies, goji berry juice and tinctures slowed that same enzyme down. If that happened in a real person, esomeprazole could clear a little slower and build up slightly, meaning a bit more drug effect and possibly more of its usual mild side effects like headache, nausea, gas, or diarrhea.
I'd keep this in perspective, though. This is only a theoretical concern from test-tube research, and it hasn't been seen in people. Eating goji berries as food is very unlikely to cause trouble. If you're using concentrated goji supplements regularly, just give your pharmacist or doctor a heads-up so they can keep an eye on things.

In vitro research shows that goji berry juice inhibits CYP3A4 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP3A4 substrates. However, this has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Nexium (esomeprazole). Your liver helps clear this medicine, partly through an enzyme called CYP3A4. Lab studies suggest goji berry might slow that enzyme down a little. In theory, that could let esomeprazole linger a bit longer in your body, which might slightly nudge up its effects or side effects like headache, nausea, gas, or mild stomach upset.
The important part: this comes from test-tube research only, and it has not been seen in real people. Eating goji berries as a food is very unlikely to cause any problem. If you're using concentrated goji supplements alongside Nexium regularly, just mention it to your pharmacist or doctor so they can keep an eye on things. Don't change anything on your own.
