Why this applies to Flecainide

Flecainide falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Goji interacts with.

Theoretically, goji berry might inhibit CYP2D6 and reduce metabolism of CYP2D6 substrates.
In vitro research shows that goji berry juice inhibits CYP2D6 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP2D6 substrates. However, this has not been reported in humans.

Here's the gist for goji and Flecainide. Your body relies on a liver enzyme called CYP2D6 to break flecainide down and clear it out. Lab studies suggest goji berry juice may slow that enzyme. If it does, flecainide could clear a little more slowly and build up in your system, which might nudge its effects and side effects upward (things like dizziness, blurred vision, a slower or more irregular heartbeat, or feeling faint).

That said, this is theoretical. It's only been seen in test tubes, never reported in real people, and everyday culinary amounts of goji are unlikely to matter. Still, flecainide is a heart-rhythm drug with a narrow safety window, so before adding concentrated goji supplements, check with your pharmacist or doctor first.

Theoretically, goji berry might increase the levels and clinical effects of flecainide.
In one case report, a 75-year-old patient stable on flecainide and warfarin presented to the emergency room with fainting and pleomorphic arrhythmia caused by flecainide toxicity. Flecainide toxicity was attributed to drinking 1-2 glasses of concentrated goji tea daily for 2 weeks. Theoretically, goji may have inhibited the cytochrome P450 2D6 (CYP2D6) metabolism of flecainide.

Here's the situation with goji and your flecainide. Flecainide is cleared from your body partly by a liver enzyme called CYP2D6. Goji appears to slow that enzyme down, which means flecainide can clear more slowly and build up to higher levels than expected. Flecainide is what we call a narrow-margin heart rhythm drug, so even a modest rise in its level can matter.

If flecainide climbs too high, it can actually cause the very thing it's meant to prevent: a dangerous irregular heartbeat, along with dizziness or fainting. I want to be honest though, this comes from a single case report involving daily concentrated goji tea, so it's more of a theoretical caution than a proven rule. Occasional goji berries as food are unlikely to be a problem. Please run any concentrated goji product past your doctor or pharmacist first.

About Flecainide

What Is Flecainide Used For?

Brand name: Tambocor

Flecainide Acetate tablets are used to prevent episodes of certain abnormal heart rhythms. In patients without structural heart disease, it is approved to prevent paroxysmal supraventricular tachycardia (PSVT — episodes of rapid heart rate starting above the ventricles) and paroxysmal atrial fibrillation/flutter (PAF — intermittent episodes of irregular, rapid heart rhythm in the upper chambers), when these cause disabling symptoms.

Flecainide Acetate tablets are also used to prevent life-threatening ventricular arrhythmias — dangerous rapid rhythms starting in the lower chambers of the heart, such as sustained ventricular tachycardia. It is not recommended for less serious ventricular arrhythmias, for patients with a recent heart attack, or for chronic (ongoing, persistent) atrial fibrillation.
