Here's the practical takeaway for your Avalide, which combines a water pill (hydrochlorothiazide) with a blood pressure medicine (irbesartan). The concern on file is only theoretical, and it points to goji root bark, not the goji fruit or fruit juice that most people eat or buy. Lab and animal studies hint that the root bark might lower blood pressure on its own, so stacking it on top of your medication could, in theory, push your pressure down a bit too far.

If that ever happened, you might feel lightheaded, dizzy, tired, or faint when standing up quickly. Good news: goji fruit has not lowered blood pressure in human studies, so enjoying the berries as food is very unlikely to cause a problem. Before starting any concentrated goji root supplement, just check with your pharmacist or doctor.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

Hydrochlorothiazide, Irbesartan also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Theoretically, goji berry might inhibit CYP2C9 and reduce metabolism of CYP2C9 substrates.
In vitro research shows that goji berry tincture and juice inhibit CYP2C9 enzymes. Additionally, multiple case reports suggest that goji berry concentrated tea and juice inhibit the metabolism of warfarin, a CYP2C9 substrate. Concomitant use with goji may decrease metabolism and increase levels of CYP2C9 substrates.

Here's the thing with Avalide (irbesartan plus hydrochlorothiazide) and goji. Irbesartan is partly broken down by a liver enzyme called CYP2C9, and goji berry appears to slow that enzyme down a bit. In theory, that could let more irbesartan build up, which might nudge your blood pressure lower than expected. You could notice things like lightheadedness, dizziness when standing, or feeling unusually tired.

I want to be honest about the evidence though. This is mostly lab studies and a few case reports with warfarin, not solid proof with irbesartan. A few goji berries as food are very unlikely to matter. Concentrated goji juices, teas, or supplements are the bigger question. Just run it by your pharmacist or doctor first.
