Hydrocodone falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Goji interacts with.

Theoretically, goji berry might inhibit CYP2D6 and reduce metabolism of CYP2D6 substrates.
In vitro research shows that goji berry juice inhibits CYP2D6 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP2D6 substrates. However, this has not been reported in humans.

Here's the wrinkle with hydrocodone: it's actually a prodrug in part, meaning your liver uses an enzyme called CYP2D6 to convert it into hydromorphone, which is a much stronger painkiller. Goji berry has shown, in lab dish studies only, that it might block that same enzyme. If that blocking happened in your body, you'd convert less hydrocodone into the potent form, so you might get less pain relief than expected.

I want to be honest about the strength of this. It comes from test-tube research and has never been seen in real people, so it's mostly theoretical. Everyday culinary amounts of goji berries are unlikely to matter much. Concentrated goji supplements are the bigger question mark. If your pain control changes, check in with your pharmacist or doctor before adjusting anything.

Here's the gist for hydrocodone: your liver uses an enzyme called CYP3A4 to help process this opioid. Lab tests (not real people yet) suggest goji berry juice might slow that enzyme down a bit. If that happened in your body, hydrocodone could clear a little slower and build up, which in theory could mean more drowsiness, dizziness, constipation, or slowed breathing.

The good news: this is only a theoretical concern from test-tube studies, and it has never been reported in actual patients. Eating goji berries as a food is very unlikely to cause trouble. Concentrated goji supplements are the bigger question mark. Since hydrocodone is a powerful pain medicine, it's worth a quick chat with your pharmacist or doctor before adding goji supplements.
