Irbesartan, Hydrochlorothiazide falls under ANTIHYPERTENSIVE DRUGS — the group Goji interacts with.

Theoretically, concomitant use of goji root bark, but not goji fruit, with antihypertensive drugs might have additive effects.
Animal and in vitro research suggest that goji root bark has hypotensive effects. However, goji fruit juice does not appear to reduce systolic or diastolic blood pressure in humans.

Here's the key thing to know: your CoAprovel combines irbesartan (which relaxes blood vessels) with hydrochlorothiazide (a water pill), and both already work to lower blood pressure. The concern on file involves goji root bark, not the berries or juice you'd normally eat. In lab and animal studies, goji root bark seemed to lower blood pressure on its own, so in theory stacking it on top of your medication could push your pressure a bit too low.

That said, this is theoretical only, and goji fruit juice did not lower blood pressure in people. Eating goji berries as food is very unlikely to cause trouble. If you're using a concentrated root bark product, watch for dizziness, lightheadedness, or feeling faint when standing, and check in with your pharmacist or doctor first.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

Irbesartan, Hydrochlorothiazide also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Theoretically, goji berry might inhibit CYP2C9 and reduce metabolism of CYP2C9 substrates.
In vitro research shows that goji berry tincture and juice inhibit CYP2C9 enzymes. Additionally, multiple case reports suggest that goji berry concentrated tea and juice inhibit the metabolism of warfarin, a CYP2C9 substrate. Concomitant use with goji may decrease metabolism and increase levels of CYP2C9 substrates.

Here's the gist with CoAprovel (irbesartan plus hydrochlorothiazide). One of its ingredients, irbesartan, is partly cleared by a liver enzyme called CYP2C9. Goji berry may gently slow that enzyme down, so in theory irbesartan could hang around a little longer and have a slightly stronger effect. With a blood pressure medicine, that might mean more dizziness, lightheadedness, or feeling faint when you stand up.

I want to be honest about how soft this evidence is. It comes from lab studies and a few case reports involving warfarin, not from irbesartan itself, and a handful of goji berries in your food is very unlikely to matter. Concentrated goji teas, juices, or supplement doses are where I'd be more cautious. If you want to use those regularly, check with your pharmacist or doctor first.
