Goji and Perindopril Arginine, Amlodipine

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Goji interacts with whole categories of medication. Perindopril Arginine, Amlodipine falls into 2 of those categories, so it can interact with Goji in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

ANTIHYPERTENSIVE DRUGS

1 of 2

Why this applies to Perindopril Arginine, Amlodipine

Perindopril Arginine, Amlodipine falls under ANTIHYPERTENSIVE DRUGS — the group Goji interacts with.

Theoretically, concomitant use of goji root bark, but not goji fruit, with antihypertensive drugs might have additive effects.
Animal and in vitro research suggest that goji root bark has hypotensive effects. However, goji fruit juice does not appear to reduce systolic or diastolic blood pressure in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the gist for Prestalia (perindopril plus amlodipine), which is a combo pill that already lowers your blood pressure. The concern on file isn't really about goji berries at all. It's about goji root bark, a different part of the plant that lab and animal studies suggest may have its own blood-pressure-lowering effect. Stacked on top of your medicine, that could in theory add up and drop your pressure too low, leaving you dizzy, lightheaded, or tired, especially when you stand up quickly.

The good news: this is only a theoretical concern, and human studies of goji fruit juice didn't lower blood pressure. So eating goji berries is very unlikely to be a problem. If you're considering a concentrated root bark supplement, just check with your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 2

Perindopril Arginine, Amlodipine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, goji berry might inhibit CYP3A4 and reduce metabolism of CYP3A4 substrates.
In vitro research shows that goji berry juice inhibits CYP3A4 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP3A4 substrates. However, this has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Prestalia combines two blood pressure medicines. The amlodipine half is cleared mainly by a liver enzyme called CYP3A4. Lab studies suggest goji juice may slow that enzyme down a bit. In theory, that could let amlodipine build up slightly, which might nudge its effects up: think more lightheadedness, flushing, headache, or some ankle swelling. The perindopril side isn't really affected by this enzyme.

Here's the honest part: this comes only from test-tube research, not from people, so it's a maybe, not a sure thing. Eating goji berries as a food is very unlikely to cause trouble. Concentrated goji supplements are where I'd be a little more careful. Just keep an eye on how you feel and check with your pharmacist or doctor before adding them.
