Zanubrutinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goji interacts with.

Theoretically, goji berry might inhibit CYP3A4 and reduce metabolism of CYP3A4 substrates.
In vitro research shows that goji berry juice inhibits CYP3A4 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP3A4 substrates. However, this has not been reported in humans.

Here's the simple version. Your body clears zanubrutinib (Brukinsa) mainly through a liver enzyme called CYP3A4. Lab studies suggest goji berry juice may slow that same enzyme down. If it does, your body could clear zanubrutinib a little more slowly, letting levels creep up. That could mean more of the usual side effects, like bruising or bleeding, low blood counts, infections, or feeling extra tired.

The honest part: this is only seen in test-tube research so far, never confirmed in real people, so I don't want to alarm you. A few goji berries in food are unlikely to be a problem. Concentrated goji supplements or daily goji juice are a different story. Since zanubrutinib is a serious cancer medicine, please run it past your oncologist or pharmacist before adding goji.

What Is Zanubrutinib Used For?

Brand name: Brukinsa

Brukinsa (zanubrutinib) is used to treat adult patients with several blood cancers. These include chronic lymphocytic leukemia (CLL), small lymphocytic lymphoma (SLL), Waldenström's macroglobulinemia (WM), mantle cell lymphoma (MCL) after at least one prior treatment, relapsed or refractory marginal zone lymphoma (MZL) after at least one prior anti-CD20 regimen, and relapsed or refractory follicular lymphoma (FL) in combination with obinutuzumab after two or more prior lines of treatment.

Some of these uses — MCL, MZL, and FL — are approved under accelerated approval, meaning they are based on tumor response rates rather than confirmed long-term survival benefit. Continued approval for those uses may depend on results from confirmatory trials.
