Brexpiprazole falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Grape interacts with.

Theoretically, grape seed extract may increase the levels of CYP2D6 substrates.
In vitro evidence suggests that grape seed extract might inhibit CYP2D6 enzymes. However, this interaction has not been reported in humans.

Here's the basic idea: your body uses a liver enzyme called CYP2D6 to help clear brexpiprazole (Rexulti) out of your system. Lab studies hint that concentrated grape seed extract might slow that enzyme down a bit. If it did, your brexpiprazole could build up slightly, which in theory might mean more of its usual effects, things like drowsiness, restlessness, dizziness, or weight changes.

The important part to hear is how weak this evidence is. It comes from test-tube work only and has never actually been seen in people. Eating grapes or drinking grape juice is not a concern. If you're thinking about a grape seed extract supplement, just run it by your pharmacist or doctor first so they can keep an eye on things.

It is unclear if grape seed extract inhibits or induces CYP3A4; research is conflicting.
In vitro evidence suggests that grape seed extract might inhibit CYP3A4 enzymes. However, evidence from animal research shows that grape seed extract may induce CYP3A4 in the liver. So far, these interactions have not been reported in humans.

Here's the honest picture with grape seed extract and Rexulti (brexpiprazole). Your body clears brexpiprazole largely through a liver enzyme called CYP3A4, and grape seed extract may nudge that enzyme one way or the other. The catch is that the science is genuinely mixed. Lab (test tube) studies hint it might slow the enzyme, which could let brexpiprazole build up and bring more drowsiness, restlessness, or weight changes. Animal studies suggest the opposite, that it might speed up clearance and weaken the benefit.

Because brexpiprazole isn't a prodrug, it works on its own without needing activation. The good news is this has never actually been reported in people, so it's mostly theoretical. Still, occasional grapes as food are no worry. If you're considering a concentrated supplement, check with your pharmacist or doctor first.

What Is Brexpiprazole Used For?

Brand name: Rexulti

Brexpiprazole tablets are approved for two uses in adults: as an add-on (adjunctive) treatment alongside antidepressants for major depressive disorder (MDD), and as a treatment for schizophrenia. These uses apply to both the generic brexpiprazole tablets and Rexulti.

Rexulti has one additional approved use: treating agitation associated with dementia due to Alzheimer's disease in adults. Rexulti is also approved for schizophrenia in adolescents ages 13 and older. Note that Rexulti is not approved for dementia-related psychosis without agitation from Alzheimer's disease, and it is not meant to be used on an as-needed basis for Alzheimer's-related agitation.
