Erythromycin Stearate falls under CYTOCHROME P450 1A2 (CYP1A2) INHIBITORS — the group Green Tea interacts with.
Green tea contains caffeine. Caffeine is metabolized by cytochrome P450 1A2 (CYP1A2),. Theoretically, drugs that inhibit CYP1A2 may decrease the clearance rate of caffeine from green tea and increase caffeine levels.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to understand about this pairing: it's really about the caffeine in green tea, not the antibiotic itself. Erythromycin can gently slow down a liver enzyme called CYP1A2, which is the same enzyme your body uses to clear caffeine. So while you're taking erythromycin, the caffeine from green tea may hang around longer than usual.
In plain terms, your usual cup of green tea might hit a little harder. You could feel jittery, have trouble sleeping, notice a racing heartbeat, or feel a bit anxious or shaky. This is theoretical and based on how the drugs behave, so it's not a strong warning. A cup or two is unlikely to cause trouble for most folks. If you take green tea extract or drink a lot, mention it to your pharmacist or doctor.

P-GLYCOPROTEIN SUBSTRATES

In vitro research and case reports suggest that green tea inhibits drug efflux by P-gp, potentially increasing serum levels of P-gp substrates. Case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking green tea and certain P-gp substrates.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for your erythromycin: your body uses a tiny pump called P-glycoprotein to help move this antibiotic out of cells and clear it along. Green tea may slow that pump down a bit, which in theory could let a little more erythromycin hang around in your system than usual.
If that happened, you might notice more of erythromycin's typical complaints, like an upset stomach, nausea, cramping, or diarrhea. I want to be honest, though: this is based on lab studies and a few scattered case reports, so it's possible but not well proven. A normal cup or two of green tea is very unlikely to cause trouble. Concentrated green tea extract pills are where I'd be more cautious. Either way, mention what you're taking to your pharmacist or doctor.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

In vitro and in vivo research suggests that green tea can inhibit intestinal CYP3A and induce hepatic CYP3A4 enzymes. However, this effect is unlikely to be clinically significant, as green tea does not appear to affect CYP3A4 activity in humans.

Here's the good news if you enjoy a cup of green tea while taking erythromycin: this pairing is very unlikely to cause you any trouble. Erythromycin is handled by an enzyme system called CYP3A4, and lab and animal studies hint that green tea might nudge that enzyme in your gut or liver. But in actual people, green tea hasn't shown a meaningful effect on how this drug works.
Erythromycin is an active antibiotic (not a prodrug), so it doesn't need to be switched on by an enzyme to do its job. Drinking normal amounts of tea shouldn't change how well it fights your infection or its usual side effects like stomach upset or nausea. Concentrated green tea extract pills are stronger, so if you take those, just mention it to your pharmacist or doctor.

About Erythromycin
What Is Erythromycin Used For?
Brand names: Erythro-S, Erythrocin, Ethril
Erythromycin treats bacterial infections caused by susceptible organisms. Oral and intravenous forms are used for upper and lower respiratory tract infections, skin and soft-tissue infections, pertussis (whooping cough), Legionnaires' disease, sexually transmitted infections (gonorrhea, syphilis, chlamydia), listeriosis, and other conditions. Erythrocin Lactobionate IV and erythromycin lactobionate injection are specifically for patients who cannot take oral medication or who need rapid, high blood levels due to severe infection — IV therapy should be switched to oral as soon as appropriate.
Erythromycin topical solution 2% and Ery 2% Pads (swabs) are indicated only for the topical treatment of acne vulgaris (acne) and are applied directly to the skin — they are not used for internal infections.
