Metformin, Sitagliptin falls under ANTIDIABETES DRUGS — the group Gymnema interacts with.

Theoretically, taking gymnema with antidiabetes drugs might increase the risk of hypoglycemia.
Gymnema reduces blood glucose levels in some human and animal research. In human studies, it has been shown to enhance the blood glucose lowering effects of hypoglycemic drugs. However, other research in adults with prediabetes or metabolic syndrome suggests that gymnema does not reduce fasting levels of blood glucose. Until more is known, monitor blood glucose levels closely.

Janumet combines two diabetes medicines, metformin and sitagliptin, and both work to bring your blood sugar down. Gymnema also seems to nudge blood sugar lower on its own. So the concern here isn't about the herb changing how your body breaks the drug down. It's simpler than that: you'd have two things pushing in the same direction, and your blood sugar could dip too far.

If that happens, you might feel shaky, sweaty, dizzy, lightheaded, or unusually hungry. The evidence is still fairly early and a bit mixed, so this isn't a sure thing for everyone. Still, if you decide to try gymnema, it's worth checking your blood sugar more often and talking with your pharmacist or doctor first so you don't get caught off guard.
