Gymnema + Sacubitril, Valsartan
Sacubitril, Valsartan brand name: Entresto.
Gymnema has a moderate interaction with Sacubitril, Valsartan. Theoretically, gymnema might increase or decrease levels of drugs metabolized by CYP2C9.

Here is the interaction we found between these two, with the evidence behind it.
CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES
Why this applies to Sacubitril, Valsartan
Sacubitril, Valsartan falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Gymnema interacts with.
Animal research shows that gymnema can induce the CYP2C9 enzyme. In one animal study, gymnema caused a 2.4-fold increase in the clearance of tolbutamide, a CYP2C9 substrate, in rats. In vitro research also shows that gymnema can inhibit CYP2C9.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest picture with Entresto (sacubitril/valsartan) and gymnema. The valsartan part can be handled in a small way by a liver enzyme called CYP2C9, and gymnema seems to nudge that enzyme. The catch is that lab and animal studies point in two opposite directions: in rats gymnema sped up an enzyme (which could lower drug levels), while test-tube work suggests it might slow it down (which could raise levels). So we honestly can't say which way it would go in a real person.
The good news: this is theoretical, based only on lab and animal data, so I wouldn't lose sleep over it. Still, gymnema can also lower blood sugar on its own, and Entresto can affect your blood pressure. Please check with your pharmacist or doctor before combining them.

From our pharmacist’s overview on the Sacubitril, Valsartan page
“Entresto (sacubitril and valsartan) is a heart failure medicine, and heart failure patients tend to be heavy supplement users, reaching for fish oil, potassium, and herbal blood-pressure remedies. That instinct carries real risk here.”

About Gymnema
What is Gymnema?
- Blood sugar support
- Type 2 diabetes
- Sugar cravings
- Weight management
- Cholesterol support
Gymnema is an Ayurvedic herb best known for possibly helping lower blood sugar and reducing the taste of sweetness on the tongue. Some early human studies are encouraging for blood sugar support, but the evidence is still limited and not strong enough to replace standard treatment. If you have diabetes or take other medicines, talk with your pharmacist or doctor before using it.
About Valsartan and Sacubitril
What Is Valsartan and Sacubitril Used For?
Brand name: Entresto
Sacubitril and valsartan is used to reduce the risk of cardiovascular death and hospitalization for heart failure in adult patients with chronic heart failure. The benefit is most clearly seen in patients whose heart pumps with a lower-than-normal ejection fraction (the percentage of blood the heart pushes out with each beat).
It is also approved to treat symptomatic heart failure with reduced left ventricular function in children aged 1 year and older. In pediatric patients, it lowers a blood marker of heart stress (NT-proBNP) and is expected to improve heart outcomes.
