Acalabrutinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Melatonin interacts with.

Theoretically, melatonin might increase levels of drugs metabolized by CYP3A4.
Laboratory research shows that certain lots of melatonin inhibit CYP3A4. Theoretically, combined administration of melatonin with certain drugs metabolized by CYP3A4 may reduce the metabolism of these drugs, resulting in increased serum levels. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.

Here's the gist for Calquence (acalabrutinib): your liver uses an enzyme called CYP3A4 to break this medicine down. Some lab studies suggest melatonin might slow that same enzyme a little. If that happened in real life, acalabrutinib could clear more slowly and its levels could creep up, which might mean more of its usual side effects like headaches, bruising or bleeding, tiredness, or a higher infection risk.

I want to be honest about the strength of this, though. This concern comes only from test-tube research, not from people, and it's rated as a minor, theoretical interaction. So it's worth a quick mention to your oncologist or pharmacist rather than a reason to panic. Since acalabrutinib is a serious cancer drug, just let your care team okay any melatonin before you start it.

What Is Acalabrutinib Used For?

Brand name: Calquence

Calquence is used to treat adults with mantle cell lymphoma (MCL) — either newly diagnosed (in combination with bendamustine and rituximab, for patients who are not candidates for a stem cell transplant) or previously treated (after at least one prior therapy). It is also used to treat adults with chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL).

Both the capsule and tablet forms of Calquence are approved for all three of these uses. The specific combination regimen and treatment schedule depend on the condition being treated and what other medicines are used alongside it — your care team will map that out for you.
