Acetaminophen, Butalbital, Caffeine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Melatonin interacts with.

Theoretically, melatonin might increase levels of drugs metabolized by CYP1A2. Also, other CYP1A2 substrates might decrease the metabolism of melatonin, increasing melatonin levels.
Melatonin is metabolized in the liver primarily by the CYP2C19 and CYP1A2 enzymes. Theoretically, combined administration of melatonin with drugs metabolized by the CYP1A2 enzyme might reduce the metabolism of these drugs, resulting in increased serum levels. Conversely, some drugs metabolized by CYP1A2 may inhibit the metabolism of melatonin, resulting in increased serum levels of melatonin. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.

Here's the piece that actually matters in your headache pill: the caffeine. Caffeine is cleared by a liver enzyme called CYP1A2, and melatonin can theoretically slow that enzyme down a touch. If caffeine hangs around a little longer, you might notice more of its usual effects, like feeling jittery, a faster heartbeat, or trouble falling asleep (which is a bit counterproductive when you're taking melatonin to sleep).

I want to be honest about the strength of this one. It's based on how these substances behave on paper, not on real patient reports, so for most people it's likely minor. The butalbital and acetaminophen parts aren't really the concern here. Still, if you take this combo regularly, just run it by your pharmacist or doctor before adding melatonin.

Theoretically, taking caffeine with melatonin might increase levels of melatonin.
Some evidence suggests that caffeine consumption can decrease endogenous melatonin levels, while other evidence suggests that caffeine increases endogenous melatonin levels. When administered in combination with melatonin supplements, caffeine seems to increase melatonin effects and levels. The reason for this discrepancy is not completely clear. Part of the discrepancy may result from the fact that caffeine can inhibit melatonin synthesis as well as inhibit melatonin metabolism. By functioning as an adenosine receptor antagonist, caffeine may indirectly inhibit the synthesis of melatonin. Conversely, because melatonin and caffeine are both metabolized by cytochrome P450 1A2 (CYP1A2) enzyme, concomitant use of melatonin and caffeine may reduce the metabolism of melatonin, resulting in higher serum levels.

Here's the angle that matters: this headache medicine contains caffeine, and caffeine is the piece that talks to your melatonin. Both are cleared by the same liver enzyme (CYP1A2), so when they're on board together, caffeine can slow how fast your body breaks melatonin down. The result is that melatonin levels and its sleepy, wind-down effects may run a little higher than expected.

For most people taking Esgic or Fioricet occasionally for a tension headache, this is a mild, mostly theoretical concern based on lower-quality studies. You might just feel a touch more drowsy. Keep in mind the butalbital in this combo is already sedating, so adding melatonin could stack that effect. If you use both regularly, check with your pharmacist or doctor first.

Here's the gist with melatonin and your headache medicine (the acetaminophen, butalbital, and caffeine combo). Some lab studies suggest melatonin might slow down a liver enzyme called CYP3A4, which helps clear parts of this medication. If that enzyme works a little slower, levels of the drug could nudge upward. In theory, that might make the butalbital side a touch stronger, so you could feel extra drowsy, lightheaded, or foggy.

That said, this is very weak, test-tube-level evidence, and it's rated a minor interaction. For most people it won't cause real trouble. Still, because butalbital is sedating and melatonin also makes you sleepy, just go easy and watch for added grogginess. If you take these together regularly, mention it to your pharmacist or doctor.

This three-ingredient combination — butalbital, acetaminophen, and caffeine — is used to relieve the symptoms of tension headache (also called muscle contraction headache). All supported products, including tablets, capsules, and oral solution, share this same approved use.

It is worth knowing that evidence for using this combination to treat repeated, frequent tension headaches over time is limited. Because butalbital can be habit-forming, extended or repeated use is not recommended.
