This combination product already packs a sedating punch. Carisoprodol is a muscle relaxant that makes many people drowsy, and the codeine piece is an opioid that adds its own sleepiness on top. Melatonin sends an "it's nighttime" signal to your brain, so stacking it with these two can pile on the drowsiness.

This concern is theoretical, based on how these ingredients work rather than on strong studies, so don't panic. Still, watch for extra grogginess, dizziness, slowed thinking, or feeling unusually out of it, especially when driving. Older adults are more sensitive. If you take both, please check with your pharmacist or doctor first so they can guide the timing and dose.

This combination product mixes aspirin (a pain reliever), carisoprodol (Soma, a muscle relaxer), and codeine (an opioid). The piece that connects to melatonin here is carisoprodol, since muscle relaxers like it can lower the seizure threshold a bit, meaning the brain becomes slightly more prone to a seizure. The concern on file is theoretical: in some people, especially children with neurological conditions, melatonin has seemed to make seizures more frequent.

For most adults using this medication short-term for pain, this is a low-level, mostly on-paper worry rather than something proven. CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES

Theoretically, melatonin might increase levels of drugs metabolized by CYP2C19. Also, other CYP2C19 substrates might decrease the metabolism of melatonin, increasing melatonin levels.
Melatonin is metabolized in the liver primarily by the CYP2C19 and CYP1A2 enzymes. Theoretically, combined administration of melatonin with certain drugs metabolized by the CYP2C19 enzyme may reduce the metabolism of these drugs, resulting in increased serum levels. Conversely, some drugs metabolized by CYP2C19 may inhibit the metabolism of melatonin, resulting in increased serum levels of melatonin. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.

This combo product mixes aspirin, the muscle relaxer carisoprodol, and codeine. The interaction on file is mostly about carisoprodol, which your liver activates and clears using the CYP2C19 enzyme. The same enzyme also helps break down melatonin, so in theory the two could compete and slow each other's processing. That might leave a bit more carisoprodol (and its metabolite meprobamate) around, which could add to drowsiness, dizziness, or grogginess, especially layered on top of melatonin's own sleepy effect and codeine's sedation.

Honestly, this is a theoretical concern based on how these drugs are handled in the body, not strong real-world data. Still, since both can make you drowsy, it's worth a quick chat with your pharmacist or doctor before pairing them. Never start or stop either on your own.

Theoretically, melatonin may have anticoagulant effects and may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
There are isolated case reports of minor bleeding and decreased prothrombin activity in people taking melatonin with warfarin (Coumadin). The mechanism, if any, of this interaction is unknown. Taking melatonin orally seems to decrease coagulation activity within one hour of dosing in healthy men.

This combination product contains aspirin, which thins your blood a bit by making platelets less sticky. The concern here is the aspirin piece, not the carisoprodol (a muscle relaxer) or codeine (a pain reliever). Melatonin may very gently nudge your blood toward clotting less easily, so pairing it with aspirin could, in theory, add up to a slightly higher bleeding risk.

To be fair, this is mostly theoretical. The few real reports involved warfarin, not aspirin, and the effect was minor. Still, it is worth watching for things like easy bruising, nosebleeds, bleeding gums, or stomach upset (aspirin can already irritate the stomach). If you notice any of that, or take aspirin daily, check with your pharmacist or doctor before adding melatonin.

Here's the wrinkle with this combo product: the codeine part is what interacts with melatonin. Codeine is a prodrug, meaning it doesn't do much until your body converts it into morphine using an enzyme called CYP2D6. Some lab studies hint that melatonin might slow that same enzyme down a bit.

If melatonin blocks CYP2D6, you'd actually make less morphine, so you might get weaker pain relief rather than stronger sedation. That said, this is only test-tube research, the rating is minor, and it's far from proven in real people. Still, since this medicine already carries drowsiness from the codeine and carisoprodol, I'd run it by your pharmacist before pairing it with melatonin, just to play it safe.
