Aspirin, Omeprazole falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Melatonin interacts with.
Melatonin is metabolized in the liver primarily by the CYP2C19 and CYP1A2 enzymes. Theoretically, combined administration of melatonin with certain drugs metabolized by the CYP2C19 enzyme may reduce the metabolism of these drugs, resulting in increased serum levels. Conversely, some drugs metabolized by CYP2C19 may inhibit the metabolism of melatonin, resulting in increased serum levels of melatonin. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the gist with Yosprala, which pairs aspirin with omeprazole. The omeprazole half is cleared by a liver enzyme called CYP2C19, and melatonin uses that same pathway. On paper, melatonin could slow how fast omeprazole is broken down, nudging its levels up a bit. Going the other way, omeprazole might slow melatonin's breakdown, so you could feel a little more drowsy or groggy than expected.
I want to be honest about the strength of this one: it's theoretical, based on how these chemicals behave in the lab, not on real cases of people running into trouble. Neither drug here is a prodrug that gets weaker, so this isn't about losing your heart protection. If you'd like to use melatonin, just run it by your pharmacist or doctor first.

ANTICOAGULANT/ANTIPLATELET DRUGS

There are isolated case reports of minor bleeding and decreased prothrombin activity in people taking melatonin with warfarin (Coumadin). The mechanism, if any, of this interaction is unknown. Taking melatonin orally seems to decrease coagulation activity within one hour of dosing in healthy men.

Here's the simple version: the aspirin in Yosprala already thins your blood a bit by keeping platelets from clumping together. Melatonin may have a mild blood-thinning effect of its own, so pairing the two could, in theory, nudge your bleeding risk a little higher. In real life that might look like easier bruising, nosebleeds, bleeding gums, or a small cut that takes longer to stop.
I want to be honest about the evidence, though. This is mostly theoretical, with only a few scattered case reports (and those involved warfarin, not aspirin). For most people taking occasional melatonin for sleep, it's not a major worry. Still, since aspirin affects your platelets, it's worth a quick chat with your pharmacist or doctor before combining them, especially if you bleed or bruise easily.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Laboratory research shows that certain lots of melatonin inhibit CYP3A4. Theoretically, combined administration of melatonin with certain drugs metabolized by CYP3A4 may reduce the metabolism of these drugs, resulting in increased serum levels. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.

Here's the short version for Yosprala (the aspirin plus omeprazole combo pill): the concern is mostly on paper. Some lab research suggests certain batches of melatonin might gently slow down a liver enzyme called CYP3A4. In theory, if that enzyme helps clear a drug, the drug could build up a little. For this combo, that might nudge omeprazole levels up slightly, which isn't usually a big deal.
Honestly, this is rated minor and is based only on test-tube data, so I wouldn't lose sleep over it (pun intended). I haven't seen melatonin cause real problems with this medication in everyday use. If you ever notice unusual stomach upset or just want peace of mind, run it by your pharmacist or doctor before combining them.
