Carbidopa, Levodopa falls under SEIZURE THRESHOLD LOWERING DRUGS — the group Melatonin interacts with.
Some clinical evidence suggests that melatonin may increase the frequency of seizures in certain patients, particularly children with neurological disabilities.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist of this one: melatonin and your carbidopa/levodopa (Sinemet, Rytary) aren't fighting over the same liver enzymes, so melatonin shouldn't make your Parkinson's medicine stronger or weaker. The concern on file is different. Levodopa is one of those medicines that can slightly lower the seizure threshold, and in theory melatonin layered on top might nudge seizure risk a bit higher.
I want to be honest about how weak this signal is. It's mostly theoretical, and the real-world reports come mainly from children with serious neurological conditions, not typical adults using levodopa for Parkinson's. For most people this is very unlikely to cause a problem. Still, if you have a history of seizures, or you notice new twitching, confusion, or unusual movements, loop in your pharmacist or doctor before combining them.

What Is Levodopa and Carbidopa Used For?
Brand names: Sinemet CR, Sinemet, Rytary, Dhivy
Levodopa and carbidopa is used to treat Parkinson's disease, post-encephalitic parkinsonism (parkinsonism that develops after a brain infection), and symptomatic parkinsonism that can follow carbon monoxide or manganese poisoning. These indications are shared across the oral tablet, orally disintegrating tablet, extended-release tablet, and extended-release capsule formulations (including Sinemet, Rytary, Crexont, and generic products).
DUOPA — a carbidopa/levodopa suspension given through a tube directly into the small intestine — is specifically indicated for the treatment of motor fluctuations in patients with advanced Parkinson's disease. This is a product-specific indication and does not apply to the oral forms.
