This combo cold medicine pairs chlorpheniramine (an antihistamine) with dextromethorphan (a cough suppressant). Both can nudge the brain's seizure threshold a little lower, meaning the brain becomes slightly more prone to a seizure. The concern here is mostly theoretical and based on pharmacology, not strong human studies.

Some reports suggest melatonin may bump up seizure frequency in certain people, especially children who already have neurological conditions. For most healthy adults using both short-term, this is a low worry. The bigger everyday issue is overlapping drowsiness, since both melatonin and chlorpheniramine cause sleepiness, so you may feel extra groggy. If you have a seizure history or are giving this to a child, check with your pharmacist or doctor first.

Theoretically, combined administration of melatonin with certain drugs metabolized by the CYP2D6 enzyme may reduce the metabolism of these drugs, resulting in increased serum levels. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.

This combo cold product pairs an antihistamine (chlorpheniramine) with a cough suppressant (dextromethorphan). Dextromethorphan in particular is broken down by an enzyme called CYP2D6. Some lab research hints that certain batches of melatonin may slow that enzyme down, which could theoretically let a bit more dextromethorphan build up in your system.

If that happened, you might feel a little extra drowsy, dizzy, or foggy, and very rarely dextromethorphan can cause overstimulation. But keep this in perspective: this is a theoretical concern from test-tube studies only, and the interaction is rated minor. The bigger thing most people notice is that both melatonin and chlorpheniramine make you sleepy on their own. If you take them together, go easy and avoid driving. When in doubt, run it by your pharmacist.
