Ethinyl estradiol, Levonogestrel falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Melatonin interacts with.
Melatonin is metabolized in the liver primarily by the CYP2C19 and CYP1A2 enzymes. Theoretically, combined administration of melatonin with drugs metabolized by the CYP1A2 enzyme might reduce the metabolism of these drugs, resulting in increased serum levels. Conversely, some drugs metabolized by CYP1A2 may inhibit the metabolism of melatonin, resulting in increased serum levels of melatonin. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the gist with your birth control pill (Lybrel) and melatonin. The hormone in your pill, ethinyl estradiol, is partly handled by a liver enzyme called CYP1A2. On paper, melatonin might slow that enzyme down a little, which could let estrogen levels drift up slightly. If that happened, you might notice more of the usual estrogen-type effects, like breast tenderness, nausea, bloating, or headaches.
I want to be honest, though: this is a theoretical concern based on how these things are processed, not something proven in real patients. It interestingly runs both ways too, since your pill could also nudge melatonin levels up and make you feel groggier. For most people this is minor, but if you feel off, check in with your pharmacist or doctor rather than stopping anything.

CONTRACEPTIVE DRUGS

Contraceptive drugs can increase the levels of endogenous melatonin. Theoretically, these drugs may increase the effects and adverse effects of oral melatonin.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the thing to know about combining your birth control pill (Lybrel) with melatonin: this interaction actually runs the other direction from what you might expect. Your Lybrel does not get weaker or stronger from the melatonin. Instead, the estrogen in your pill can raise the amount of melatonin in your body and slow how fast you clear it. So the melatonin side, not the contraceptive side, is what gets boosted.
In plain terms, melatonin may hit you a little harder. You might feel more drowsy, groggy in the morning, or lightheaded than expected. This is fairly mild and mostly theoretical, so don't panic. If you feel overly sleepy, a quick chat with your pharmacist about lowering the melatonin dose makes sense.

Laboratory research shows that certain lots of melatonin inhibit CYP3A4. Theoretically, combined administration of melatonin with certain drugs metabolized by CYP3A4 may reduce the metabolism of these drugs, resulting in increased serum levels. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with your birth control pill (Lybrel contains ethinyl estradiol and levonorgestrel). Your liver uses an enzyme called CYP3A4 to help clear the estrogen part of the pill. Some lab samples of melatonin appear to slow that same enzyme down a little. In theory, that could let estrogen levels drift up slightly, which might nudge side effects like nausea, breast tenderness, headaches, or some breakthrough spotting.
I want to be honest, though: this is based only on test-tube research, not real people, and it's rated a minor, theoretical concern. Neither of these pills is a prodrug, so the direction is simply mild build-up, not reduced birth control protection. Most folks combine them without trouble. If you'd like reassurance, just run it by your pharmacist or doctor.
