Ethinyl Estradiol, Levonorgestrel falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Melatonin interacts with.

Theoretically, melatonin might increase levels of drugs metabolized by CYP1A2. Also, other CYP1A2 substrates might decrease the metabolism of melatonin, increasing melatonin levels.
Melatonin is metabolized in the liver primarily by the CYP2C19 and CYP1A2 enzymes. Theoretically, combined administration of melatonin with drugs metabolized by the CYP1A2 enzyme might reduce the metabolism of these drugs, resulting in increased serum levels. Conversely, some drugs metabolized by CYP1A2 may inhibit the metabolism of melatonin, resulting in increased serum levels of melatonin. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.

Here's the situation with your birth control pill (ethinyl estradiol with levonorgestrel) and melatonin. Part of the estrogen in your pill is cleared by a liver enzyme called CYP1A2. On paper, melatonin could slow that enzyme down a bit, which might let estrogen levels drift slightly higher. In theory that could mean a touch more of the usual estrogen-type effects, like nausea, breast tenderness, headaches, or bloating.

I want to be honest that this is a theoretical concern based only on how these compounds behave in lab settings, not real-world reports of problems. The interaction is generally considered minor in practice, and your pill is not a prodrug, so it should still work for preventing pregnancy. Still, it's worth a quick mention to your pharmacist or doctor before starting melatonin.

Here's the thing to know if you're on your birth control pill (the ethinyl estradiol and levonorgestrel combo found in products like Trivora and Triphasil) and you're also reaching for melatonin at night. This interaction runs in one direction: the pill can raise the melatonin levels in your body. So when you add a melatonin supplement on top, you may feel its effects more strongly than you'd expect.

In real life, that usually just means more drowsiness, grogginess the next morning, or feeling a bit more out of it. Your birth control still works fine and stays just as effective. This is mostly theoretical and based on lower-quality studies, so don't panic. If melatonin hits you hard, a smaller dose may be plenty. Check with your pharmacist before adjusting anything.
