Melatonin and Ethinyl Estradiol, Norgestrel

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Melatonin interacts with whole categories of medication. Ethinyl Estradiol, Norgestrel falls into 3 of those categories, so it can interact with Melatonin in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

1 of 3

Why this applies to Ethinyl Estradiol, Norgestrel

Ethinyl Estradiol, Norgestrel falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Melatonin interacts with.

Theoretically, melatonin might increase levels of drugs metabolized by CYP1A2. Also, other CYP1A2 substrates might decrease the metabolism of melatonin, increasing melatonin levels.
Melatonin is metabolized in the liver primarily by the CYP2C19 and CYP1A2 enzymes. Theoretically, combined administration of melatonin with drugs metabolized by the CYP1A2 enzyme might reduce the metabolism of these drugs, resulting in increased serum levels. Conversely, some drugs metabolized by CYP1A2 may inhibit the metabolism of melatonin, resulting in increased serum levels of melatonin. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.
Severity Moderate Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for your birth control pill (Ovral and similar combos of ethinyl estradiol and norgestrel). The estrogen part is partly cleared by a liver enzyme called CYP1A2. On paper, melatonin might slow that enzyme a little, which could let estrogen levels drift up slightly. If that happened, you might notice more of the usual estrogen-type side effects, like nausea, breast tenderness, headaches, or some bloating.

The honest part: this is a theoretical concern based on how these things are processed, not something proven in real patients. It's also worth knowing the pill can nudge melatonin levels up, so you might feel a bit more drowsy. Nothing here means you should stop either one, but check with your pharmacist or doctor if you notice new symptoms.

CONTRACEPTIVE DRUGS

2 of 3

Ethinyl Estradiol, Norgestrel also falls under CONTRACEPTIVE DRUGS.

Theoretically, taking contraceptive drugs with melatonin might increase the effects and adverse effects of melatonin.
Contraceptive drugs can increase the levels of endogenous melatonin. Theoretically, these drugs may increase the effects and adverse effects of oral melatonin.
Severity Moderate Likelihood Probable Evidence B

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to keep in mind with this combo: your birth control pill (the ethinyl estradiol and norgestrel in Lo/Ovral, Cryselle, and similar brands) doesn't get weaker or stronger from melatonin, and melatonin won't reduce how well the pill prevents pregnancy. The interaction actually runs the other way. The estrogen in your pill can slow how your body clears melatonin and can nudge up your own natural melatonin levels, so the melatonin you take may feel a bit stronger than expected.

In plain terms, you might notice more next-day grogginess, drowsiness, or a groggy, heavy-headed feeling in the morning. This is mostly theoretical and based on limited research, so it's not a reason to worry, but starting with a low melatonin dose makes sense. Check with your pharmacist if you have questions.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

3 of 3

Ethinyl Estradiol, Norgestrel also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, melatonin might increase levels of drugs metabolized by CYP3A4.
Laboratory research shows that certain lots of melatonin inhibit CYP3A4. Theoretically, combined administration of melatonin with certain drugs metabolized by CYP3A4 may reduce the metabolism of these drugs, resulting in increased serum levels. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the short version on combining melatonin with your birth control pill (the ethinyl estradiol and norgestrel combo). Your body clears the estrogen part of this pill using a liver enzyme called CYP3A4. In lab dish studies, certain batches of melatonin slowed that enzyme down a little. In theory, that could let estrogen levels nudge up slightly, which might mean more of the usual estrogen-type effects like nausea, breast tenderness, headache, or bloating.

The reassuring part: this is rated minor and is based only on test-tube research, not real people. For most folks taking melatonin short-term for sleep or jet lag, this isn't expected to cause trouble or change how well your pill prevents pregnancy. Still, it's worth a quick mention to your pharmacist or doctor, especially if you take melatonin regularly.

About Melatonin

What is Melatonin?

Melatonin
  • Trouble falling asleep (insomnia)
  • Jet lag
  • Delayed sleep phase syndrome
  • Non-24-hour sleep-wake disorder
  • Shift-work sleep problems
  • Sleep problems in some children with conditions like autism or ADHD

Melatonin is a hormone your body makes naturally to help control your sleep-wake cycle, and the supplement form is widely used to help with sleep timing problems and jet lag. The evidence is strongest for certain circadian (body-clock) disorders and is more mixed for everyday insomnia. It is generally well tolerated for short-term use, but talk with your pharmacist or doctor before starting, especially for children or during pregnancy.
