Fondaparinux falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Melatonin interacts with.

Theoretically, melatonin may have anticoagulant effects and may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
There are isolated case reports of minor bleeding and decreased prothrombin activity in people taking melatonin with warfarin (Coumadin). The mechanism, if any, of this interaction is unknown. Taking melatonin orally seems to decrease coagulation activity within one hour of dosing in healthy men.

Here's the gist: fondaparinux (Arixtra) is already a strong blood thinner, and there's a theory that melatonin can mildly nudge your blood toward clotting less, too. Stack the two and you could, in theory, tip a bit further toward bleeding. With a factor Xa blocker like this, that might look like easy bruising, nosebleeds, bleeding gums, or blood in your urine or stool.

I want to be honest about the strength of this, though. The concern is mostly theoretical, with only a few scattered case reports (mostly involving warfarin, not fondaparinux). It's not a sure thing. Still, since fondaparinux is serious medicine, please run any melatonin use past your pharmacist or doctor first, and report unusual bruising or bleeding right away.

Melatonin is a hormone your body makes naturally to help control your sleep-wake cycle, and the supplement form is widely used to help with sleep timing problems and jet lag. The evidence is strongest for certain circadian (body-clock) disorders and is more mixed for everyday insomnia. It is generally well tolerated for short-term use, but talk with your pharmacist or doctor before starting, especially for children or during pregnancy.

About Fondaparinux Injection

What Is Fondaparinux Injection Used For?

Brand name: Arixtra

Fondaparinux sodium injection is used to prevent deep vein thrombosis (DVT) — blood clots that form in the legs — in adults who are having hip fracture surgery, hip replacement surgery, knee replacement surgery, or abdominal surgery. Preventing these clots also lowers the risk of a clot traveling to the lungs (pulmonary embolism, or PE).

It is also used to treat acute DVT and acute pulmonary embolism in adults, typically started alongside warfarin. In children aged 1 year and older who weigh at least 10 kg (about 22 lbs), fondaparinux sodium injection is approved to treat venous thromboembolism (VTE) — blood clots in the veins.
