Here's the idea behind this one. Melatonin can give your immune system a little nudge to be more active. Fostamatinib (Tavalisse) works in the opposite direction for immune thrombocytopenia, calming the immune attack on your platelets so fewer get destroyed. So in theory, regularly taking melatonin could work against what fostamatinib is trying to do, which might mean your platelet counts don't climb the way you'd hope.

I want to be honest that this is mostly a theoretical concern, and the evidence is on the weaker side. It doesn't mean melatonin will wreck your treatment. But since fostamatinib is prescribed when other options haven't worked, it's worth a quick chat with your pharmacist or doctor before adding melatonin, so they can keep an eye on your platelet numbers.

Fostamatinib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, melatonin might increase levels of drugs metabolized by CYP3A4.
Laboratory research shows that certain lots of melatonin inhibit CYP3A4. Theoretically, combined administration of melatonin with certain drugs metabolized by CYP3A4 may reduce the metabolism of these drugs, resulting in increased serum levels. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.

Here's the gist for these two together. Your liver uses an enzyme called CYP3A4 to help clear fostamatinib's active form (R406) from your body. Some lab studies suggest melatonin might mildly slow that same enzyme down. In theory, that could let a little more fostamatinib hang around, which might nudge up its usual side effects like diarrhea, high blood pressure, nausea, or higher liver enzyme readings.

I want to be honest, though: this is theoretical, based only on test-tube research, not real patient cases, and it's rated minor. Fostamatinib is not a prodrug here, so the direction is a slight increase in effect, not a loss of it. No need to panic. Just mention your melatonin use to your pharmacist or doctor so they can keep an eye on your blood pressure and labs.

What Is Fostamatinib Used For?

Brand name: Tavalisse

Tavalisse (fostamatinib) is used to treat thrombocytopenia — dangerously low platelet counts — in adults with chronic immune thrombocytopenia (ITP). It is specifically for people whose platelet counts have not improved enough with at least one earlier ITP treatment.

The goal is to raise platelet counts high enough to reduce the risk of serious bleeding. If platelet counts do not respond sufficiently after 12 weeks, the medication is typically stopped.
