Why this applies to Metformin
Metformin falls under ANTIDIABETES DRUGS — the group Melatonin interacts with.
Some clinical research shows that melatonin reduces levels of fasting blood glucose and improves glycemic control. However, other research suggests that melatonin might impair glucose utilization and increase insulin resistance, while other research has found no effect on glucose levels. Until more is known, use melatonin cautiously in combination with antidiabetes drugs.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest picture with melatonin and your metformin: the research is genuinely mixed. Some studies suggest melatonin may lower fasting blood sugar a bit, which could add to what metformin is already doing. Other studies hint it might actually nudge blood sugar the other way by increasing insulin resistance, and still others show no real change at all. So this isn't a clear-cut, predictable interaction.
Because melatonin doesn't change how your liver breaks down metformin, the concern is mainly the combined effect on your numbers, not a buildup of the drug. The practical worry is occasional low blood sugar (shakiness, sweating, dizziness, confusion). If you'd like to add melatonin for sleep, that's reasonable for most folks, but check with your pharmacist or doctor first and keep an eye on your readings.

Metformin hydrochloride tablets and oral solution are approved for adults and children 10 years and older. Metformin hydrochloride extended-release tablets — including brands such as Fortamet, Glumetza, and Metformin Hydrochloride Extended Release — are approved for adults only.
