Why this applies to Darolutamide

Darolutamide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Your body uses an enzyme called CYP3A4 to help clear darolutamide (Nubeqa) out of your system. Milk thistle might slow that enzyme down a little, which in theory could let darolutamide build up and lead to more side effects, like fatigue, rash, pain in your arms or legs, or changes in blood counts.

That said, the evidence here is weak. The lab studies disagree with each other, and careful human studies found that milk thistle did not meaningfully change how similar drugs are processed. There are only a handful of older case reports involving other cancer medicines. So this is unlikely to cause a real problem, but since darolutamide treats prostate cancer, it is smart to check with your oncologist or pharmacist before adding milk thistle.

P-GLYCOPROTEIN SUBSTRATES

Darolutamide also falls under P-GLYCOPROTEIN SUBSTRATES.

Theoretically, milk thistle might increase the absorption of P-glycoprotein substrates. However, this effect does not seem to be clinically significant.
In vitro research shows that milk thistle can inhibit P-glycoprotein activity and 1 case report from the World Health Organization (WHO) adverse drug reaction database describes increased abdominal pain in a patient taking milk thistle and the cancer medication vincristine, a P-glycoprotein substrate, though this patient was also taking methotrexate. However, a small pharmacokinetic study in healthy volunteers shows that taking milk thistle (Enzymatic Therapy Inc.) 900 mg, standardized to 80% silymarin, in 3 divided doses daily for 14 days does not affect absorption of digoxin, a P-glycoprotein substrate.

Here's the gist for darolutamide (Nubeqa): it relies partly on a cellular pump called P-glycoprotein to control how much gets absorbed and moved around your body. Milk thistle, in lab studies, can gently slow that pump down. The theory is that this could let a little more darolutamide into your system, which might mean slightly stronger effects or side effects like tiredness, rash, or aches.

But the real-world evidence here is reassuring. This is rated a minor, unlikely interaction. Human studies with similar pump-dependent drugs (like digoxin) showed no meaningful change. So I wouldn't lose sleep over it. Still, since darolutamide treats a serious condition, just give your pharmacist or oncologist a heads-up before adding milk thistle so they can keep an eye on things.

About Darolutamide

What Is Darolutamide Used For?

Brand name: Nubeqa

Nubeqa (darolutamide) tablets are approved for adult men with two forms of advanced prostate cancer. The first is non-metastatic castration-resistant prostate cancer (nmCRPC) — cancer that keeps growing despite hormone-lowering therapy and has not yet spread to other parts of the body. The second is metastatic castration-sensitive prostate cancer (mCSPC) — cancer that has spread but still responds to hormone-lowering treatment.

For mCSPC, Nubeqa can be used alone or in combination with docetaxel (a chemotherapy medicine). In all cases, patients also receive a gonadotropin-releasing hormone (GnRH) agonist or antagonist — a medicine that lowers testosterone — or have already had surgery to remove the testicles.
