Letrozole falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the gist for letrozole: your body partly relies on a liver enzyme called CYP3A4 to break it down, and milk thistle has been studied for possibly slowing that same enzyme. If it did slow it, letrozole could build up a bit, which in theory might mean more of its usual side effects like hot flashes, joint or bone aches, tiredness, or headaches.

The good news is the evidence here is weak. Most careful human studies show milk thistle does not meaningfully change CYP3A4 activity, and the worry comes mainly from lab tests and a handful of scattered case reports with other cancer drugs, not letrozole itself. Still, since this matters for breast cancer treatment, it's worth a quick chat with your oncologist or pharmacist before adding it.

Milk thistle is a popular herbal supplement most often used for liver health, and its main active component is a group of compounds called silymarin. About Letrozole

What Is Letrozole Used For?

Brand name: Femara

Letrozole (brand name Femara, or available as generic letrozole tablets) is used to treat breast cancer in postmenopausal women whose tumors are hormone receptor positive — meaning the cancer feeds on estrogen. It is used in several situations: as an adjuvant treatment (therapy given after primary treatment to reduce the risk of the cancer returning) for early breast cancer, and as extended adjuvant treatment for women who have already completed five years of tamoxifen therapy.

Femara and letrozole tablets are also used as first-line or second-line treatment for locally advanced or metastatic (spread to other areas) breast cancer in postmenopausal women, including cases where the cancer has progressed after other anti-estrogen therapy.
