Mefenamic Acid falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP2C9; research is conflicting.
In vitro research suggests that milk thistle might inhibit CYP2C9. Additionally, 3 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP2C9 substrates, including imatinib and capecitabine. However, contradictory clinical research shows that milk thistle extract does not inhibit CYP2C9 or significantly affect levels of the CYP2C9 substrate tolbutamide. Differences in results could be due to differences in dosages or formulations utilized.

Here's the gist: your body uses an enzyme called CYP2C9 to clear mefenamic acid (Ponstel). There's a theory that milk thistle might slow that enzyme down a bit. If that happened, mefenamic acid could linger longer and build up, which in theory could nudge up the usual NSAID side effects like stomach upset, heartburn, or a higher chance of stomach irritation.

The reassuring part is that the evidence here is weak and conflicting. Some lab studies hint at this, but better human studies showed milk thistle didn't meaningfully block CYP2C9. We rate this as minor and unlikely. Still, since mefenamic acid is hard on the stomach, it's worth a quick mention to your pharmacist or doctor before pairing them.

Mefenamic acid capsules are approved to manage mild to moderate pain in adults and adolescents 14 years of age and older, but only for short-term use — no longer than 7 days.

They are also approved to treat primary dysmenorrhea (painful menstrual periods), started at the onset of bleeding and associated symptoms.
