Why this applies to Osimertinib Mesylate

Osimertinib Mesylate falls under P-GLYCOPROTEIN SUBSTRATES — the group Milk Thistle interacts with.

Theoretically, milk thistle might increase the absorption of P-glycoprotein substrates. However, this effect does not seem to be clinically significant.
In vitro research shows that milk thistle can inhibit P-glycoprotein activity and 1 case report from the World Health Organization (WHO) adverse drug reaction database describes increased abdominal pain in a patient taking milk thistle and the cancer medication vincristine, a P-glycoprotein substrate, though this patient was also taking methotrexate. However, a small pharmacokinetic study in healthy volunteers shows that taking milk thistle (Enzymatic Therapy Inc.) 900 mg, standardized to 80% silymarin, in 3 divided doses daily for 14 days does not affect absorption of digoxin, a P-glycoprotein substrate.

Here is the gist for Tagrisso (osimertinib) and milk thistle. Osimertinib is carried in and out of your gut cells by a transporter called P-glycoprotein. In a test tube, milk thistle can slow that transporter down, and the theory is that this could let a bit more osimertinib get absorbed. If that happened, you might notice more of its usual side effects, like diarrhea, mouth sores, dry skin, or nail changes.

The good news is that this concern is mostly theoretical. When researchers actually tested milk thistle against a real P-glycoprotein drug in people, they saw no meaningful change. Because osimertinib treats cancer, I would still keep your oncology team and pharmacist in the loop before adding milk thistle, rather than starting it on your own.

Osimertinib Mesylate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the situation with milk thistle and your Tagrisso (osimertinib). Your liver uses an enzyme called CYP3A4 to help process osimertinib. The concern is that milk thistle might slow that enzyme down a little, which in theory could let a bit more of the drug build up. With a cancer medicine, that could mean more side effects like diarrhea, mouth sores, rash, dry skin, or low blood counts.

The good news: the evidence here is weak. Most careful human studies show milk thistle does not really change how the body handles similar drugs. A handful of case reports raised a flag with other cancer pills, but nothing solid. Still, since this is a serious medication, please check with your oncologist or pharmacist before adding milk thistle.

About Osimertinib

What Is Osimertinib Used For?

Brand name: Tagrisso

Tagrisso is used to treat adults with non-small cell lung cancer (NSCLC) whose tumors have specific EGFR mutations — either an exon 19 deletion or an exon 21 L858R mutation — confirmed by an FDA-approved test. It is used in several settings: after surgery to reduce the risk of cancer returning (adjuvant therapy), for locally advanced stage III NSCLC that has not progressed after chemoradiation, and as a first-line treatment for locally advanced or metastatic NSCLC (including in combination with pemetrexed and platinum-based chemotherapy).

Tagrisso is also approved for adults with metastatic NSCLC that carries a specific resistance mutation called T790M, when the cancer has stopped responding to another EGFR-targeted therapy.
