Why this applies to Prucalopride

Prucalopride falls under P-GLYCOPROTEIN SUBSTRATES — the group Milk Thistle interacts with.

Theoretically, milk thistle might increase the absorption of P-glycoprotein substrates. However, this effect does not seem to be clinically significant.
In vitro research shows that milk thistle can inhibit P-glycoprotein activity and 1 case report from the World Health Organization (WHO) adverse drug reaction database describes increased abdominal pain in a patient taking milk thistle and the cancer medication vincristine, a P-glycoprotein substrate, though this patient was also taking methotrexate. However, a small pharmacokinetic study in healthy volunteers shows that taking milk thistle (Enzymatic Therapy Inc.) 900 mg, standardized to 80% silymarin, in 3 divided doses daily for 14 days does not affect absorption of digoxin, a P-glycoprotein substrate.

Here's the gist for prucalopride (Motegrity, Resotran, Resolor) and milk thistle. Prucalopride is one of many medicines moved around by a transporter called P-glycoprotein. In a test tube, milk thistle can slow that transporter down, and in theory that could let a bit more prucalopride get absorbed. Since prucalopride is not a prodrug, more in your system would simply mean its usual effects might be slightly stronger, things like loose stools, cramping, nausea, or headache.

The good news: in real life this interaction looks very weak. A human study with another P-glycoprotein drug, digoxin, showed no meaningful change. So I wouldn't worry much, but if you notice extra stomach upset after adding milk thistle, give your pharmacist or doctor a quick call.

About Prucalopride

What Is Prucalopride Used For?

Brand names: Resotran, Resolor, Motegrity

Prucalopride tablets — sold under the brand name Motegrity and also available as generic prucalopride and prucalopride succinate — are indicated for the treatment of chronic idiopathic constipation (CIC) in adults. CIC is ongoing constipation with no identified underlying cause.

This indication applies to all approved oral tablet forms of prucalopride. It is not approved for use in children; a study in pediatric patients was stopped early because the medication did not show effectiveness in that age group.
