Lansoprazole, Amoxicillin, Clarithromycin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP3A4 enzymes and reduces CYP3A4 substrate metabolism.

Prevpac is a combination pack you take to clear an H. pylori stomach infection: the antibiotics amoxicillin and clarithromycin plus the acid reducer lansoprazole. Of these, clarithromycin is the one mainly handled by the CYP3A4 enzyme. Lab studies hint that Terminalia arjuna may slow down CYP3A4, which could in theory let clarithromycin build up a bit higher in your body.

If that happened, you might notice more of clarithromycin's usual effects, like an upset stomach, nausea, diarrhea, or a metallic taste. Keep in mind this is only test-tube evidence, not from people, so a real problem is far from certain. Still, since you'd be taking these together for a short, important treatment course, it's worth a quick check with your pharmacist or doctor before adding Terminalia.
