Benzhydrocodone, Acetaminophen falls under CNS DEPRESSANTS — the group Tianeptine interacts with.

Tianeptine has sedative effects; high doses may have additive effects with other CNS depressants.
Higher doses of tianeptine have opioid like effects and may have additive effects with other sedative drugs. In one case report, a 42-year-old male presented with respiratory depression and aspiration pneumonia due to concomitant use of high-dose tianeptine and alprazolam.

Here's the concern with combining tianeptine and Apadaz (benzhydrocodone with acetaminophen). Apadaz is already an opioid pain reliever, and at higher doses tianeptine acts a lot like an opioid too. Stacking the two means their sedating effects can pile on top of each other. The biggest worry is slowed or shallow breathing (respiratory depression), along with heavy drowsiness, confusion, or being hard to wake up. There's even a published case of dangerous breathing problems when high-dose tianeptine was mixed with another sedative.

This warning is based mostly on how these drugs work rather than large studies, but the risk is real, especially with the unregulated over-the-counter tianeptine products. Please don't change anything on your own. Talk with your pharmacist or doctor first.

What Is Benzhydrocodone and Acetaminophen Used For?

Brand name: Apadaz

Apadaz is indicated for the short-term management of acute pain — up to 14 days — that is severe enough to require an opioid and for which non-opioid treatments have not worked or are not expected to work. It combines benzhydrocodone (a prodrug that converts to the opioid hydrocodone) with acetaminophen.

Because of serious risks including addiction, abuse, and misuse, Apadaz is reserved for patients who have not tolerated or have not gotten adequate relief from alternative pain treatments. It should not be used for an extended period of time unless the pain remains severe enough to require an opioid and alternatives remain inadequate.
