Levorphanol falls under CNS DEPRESSANTS — the group Tianeptine interacts with.

Tianeptine has sedative effects; high doses may have additive effects with other CNS depressants.
Higher doses of tianeptine have opioid like effects and may have additive effects with other sedative drugs. In one case report, a 42-year-old male presented with respiratory depression and aspiration pneumonia due to concomitant use of high-dose tianeptine and alprazolam.

Here's the concern with mixing these two. Levorphanol is a strong opioid painkiller that already slows down your breathing, calms your nervous system, and can make you very drowsy. Tianeptine, especially at the higher doses found in those gas station products, starts acting like an opioid itself and has its own sedating, slowing effect on the body.

Put them together and those effects can stack on top of each other. That means more risk of heavy sedation, confusion, and dangerously slow or shallow breathing. A case report showed exactly this kind of trouble when high-dose tianeptine was combined with another sedative. This is based on how the drugs work rather than large studies, but I'd treat it seriously. Please talk with your doctor or pharmacist before combining anything with levorphanol.
