Why this applies to Meperidine, Promethazine
Meperidine, Promethazine falls under CNS DEPRESSANTS — the group Tianeptine interacts with.
Higher doses of tianeptine have opioid like effects and may have additive effects with other sedative drugs. In one case report, a 42-year-old male presented with respiratory depression and aspiration pneumonia due to concomitant use of high-dose tianeptine and alprazolam.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the concern with combining tianeptine and a Meprozine or Mepergan product. That medication packs two sedating ingredients: meperidine, an opioid painkiller, and promethazine, a drowsy antihistamine. Both already slow your brain and breathing down. Tianeptine, especially at the high doses found in those gas station products, behaves a lot like an opioid itself, so stacking it on top can pile on the sedation.
What that adds up to is a real risk of heavy drowsiness, confusion, very slow or shallow breathing, and even passing out. There's a reported case where high-dose tianeptine plus a sedative led to dangerous breathing trouble. This is based on how these drugs act rather than big studies, but please don't mix them without talking to your doctor or pharmacist first.
