Magnesium and Doxycycline

Here is the interaction we found between these two, with the evidence behind it.

Moderate

TETRACYCLINE ANTIBIOTICS

Why this applies to Doxycycline

Doxycycline falls under TETRACYCLINE ANTIBIOTICS — the group Magnesium interacts with.

Magnesium decreases absorption of tetracyclines.
Magnesium can form insoluble complexes with tetracyclines in the gut and decrease their absorption and antibacterial activity. Advise patients to take these drugs 1 hour before or 2 hours after magnesium supplements.
Severity Moderate Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the practical scoop on taking magnesium alongside doxycycline. When these two meet in your stomach, magnesium can grab onto the doxycycline and form a clump your body can't absorb well. The result? Less of the antibiotic actually gets into your bloodstream, which means it may not fight your infection as strongly as it should. With doxycycline, you really want full absorption so the medicine can do its job.

The good news is this is easy to manage with timing. A common approach is to take your doxycycline at least 1 hour before or 2 hours after any magnesium supplement (and similar minerals like calcium, iron, or antacids). Don't change your routine on your own, but do check with your pharmacist or doctor so you space them out correctly.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.

From our pharmacist’s overview on the Doxycycline page

“St. John's wort is the supplement I would flag first for anyone on doxycycline (Vibramycin, Doryx), because it can speed the antibiotic out of the body and leave an infection undertreated.”

About Doxycycline

What Is Doxycycline Used For?

Brand names: Vibra-Tabs, Vibramycin, Doryx, Monodox, Doxycin, Doxy, Doxylin, Doxy-D, Periostat, Doxycycline Injection, Oracea, Xyrosa

Doxycycline belongs to the tetracycline family of antibiotics. It's prescribed for many types of bacterial infections, including respiratory tract infections, sexually transmitted infections (STIs), tick-borne illnesses like Rocky Mountain spotted fever, and eye infections. It's also used to help prevent malaria and, in some cases, as a helper treatment for severe acne.

In situations where penicillin-type antibiotics can't be used — due to allergy, for example — doxycycline can step in as an alternative. It should only be used for infections that are proven or strongly suspected to be caused by bacteria.
