Magnesium and Amphotericin, Tetracycline

Here is the interaction we found between these two, with the evidence behind it.

Moderate

TETRACYCLINE ANTIBIOTICS

Why this applies to Amphotericin, Tetracycline

Amphotericin, Tetracycline falls under TETRACYCLINE ANTIBIOTICS — the group Magnesium interacts with.

Magnesium decreases absorption of tetracyclines.
Magnesium can form insoluble complexes with tetracyclines in the gut and decrease their absorption and antibacterial activity. Advise patients to take these drugs 1 hour before or 2 hours after magnesium supplements.
Severity Moderate Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: when you swallow magnesium too close to your tetracycline, the magnesium grabs onto the antibiotic in your stomach and forms a clump your body can't absorb well. The result is that less of the tetracycline actually makes it into your bloodstream, so it may not knock out your infection the way it should.

The good news is this is easy to work around. You don't have to give up your magnesium. Just space them out, taking your tetracycline about 1 hour before or 2 hours after the magnesium (this includes magnesium-containing antacids and many multivitamins). This separation problem is well known for tetracyclines, so it's worth doing. If you're unsure how to time your doses, your pharmacist can help you map it out.
