Adrafinil + Acetaminophen, Caffeine, Chlorpheniramine, Hydrocodone, Phenylephrine
Acetaminophen, Caffeine, Chlorpheniramine, Hydrocodone, Phenylephrine brand name: Hycomine Compound.
Acetaminophen, Caffeine, Chlorpheniramine, Hydrocodone, Phenylephrine falls into 3 drug categories that Adrafinil can interact with; the most serious is rated moderate. Theoretically, adrafinil might reduce the levels and clinical effects of CYP1A2 substrates.

These two interact in 3 distinct ways. Each one is explained separately below.
Why Are There 3 Interactions?
Adrafinil interacts with whole categories of medication. Acetaminophen, Caffeine, Chlorpheniramine, Hydrocodone, Phenylephrine falls into 3 of those categories, so it can interact with Adrafinil in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
1 of 3Why this applies to Acetaminophen, Caffeine, Chlorpheniramine, Hydrocodone, Phenylephrine
Acetaminophen, Caffeine, Chlorpheniramine, Hydrocodone, Phenylephrine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Adrafinil interacts with.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP1A2 activity in vitro.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version. Once your body turns adrafinil into modafinil, that modafinil may slightly speed up a liver enzyme called CYP1A2. In this combination product, the ingredient that really leans on CYP1A2 is the caffeine. If that enzyme works a little faster, your body could clear caffeine a bit quicker, so you might notice less of its pick-me-up effect.
The good news is this is only a theoretical concern based on lab studies, not real-world reports, so I wouldn't lose sleep over it. The other parts (acetaminophen, chlorpheniramine, hydrocodone, phenylephrine) aren't really affected by this pathway. Still, mixing a stimulant-type supplement with a product containing an opioid and an antihistamine is worth a quick chat with your pharmacist before combining them.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 3Acetaminophen, Caffeine, Chlorpheniramine, Hydrocodone, Phenylephrine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: once your liver turns adrafinil into modafinil, modafinil can mildly speed up a liver enzyme called CYP3A4. One ingredient in this combo cold-and-pain product, hydrocodone, relies partly on that same enzyme. So in theory, adrafinil could clear hydrocodone a little faster, which might make your pain relief feel weaker than expected.
I want to be honest about the evidence though. This is only a theoretical concern based on how these drugs work, not something proven in real patients, and the enzyme effect is described as weak. The bigger practical issue is overlap: modafinil and the caffeine here are both stimulants, while hydrocodone and chlorpheniramine cause drowsiness, so you may feel a tug-of-war. Run it by your pharmacist before combining them.

STIMULANT DRUGS
3 of 3Acetaminophen, Caffeine, Chlorpheniramine, Hydrocodone, Phenylephrine also falls under STIMULANT DRUGS.
Adrafinil is a precursor to the prescription drug modafinil, which has stimulant effects.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination product has a lot going on, but the piece that matters here is the caffeine and the phenylephrine (a decongestant). Both are mild stimulants. Adrafinil turns into modafinil in your liver, which is also a stimulant that promotes wakefulness. So stacking them together could, in theory, pile on the jittery effects: a racing or pounding heart, higher blood pressure, anxiousness, and trouble sleeping.
Keep in mind this is a theoretical concern based on how these drugs work, not something proven in studies, so I wouldn't panic. Still, the hydrocodone here can cause drowsiness while the stimulants push the opposite way, which is a messy mix. Please check with your pharmacist or doctor before pairing these.
