Adrafinil and Acetaminophen, Caffeine, Codeine, Salicylamide

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Adrafinil interacts with whole categories of medication. Acetaminophen, Caffeine, Codeine, Salicylamide falls into 3 of those categories, so it can interact with Adrafinil in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

1 of 3

Why this applies to Acetaminophen, Caffeine, Codeine, Salicylamide

Acetaminophen, Caffeine, Codeine, Salicylamide falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Adrafinil interacts with.

Theoretically, adrafinil might reduce the levels and clinical effects of CYP1A2 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP1A2 activity in vitro.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

This combo pill has a few moving parts, but the piece that matters here is the caffeine. Your liver clears caffeine using an enzyme called CYP1A2. Adrafinil turns into modafinil in your body, and lab studies suggest modafinil may speed that enzyme up a little. In theory, that could clear caffeine a bit faster, so you might notice the caffeine working less or wearing off sooner.

I want to be honest: this is a theoretical concern based on lab data, not real patient reports, so I wouldn't lose sleep over it. The acetaminophen, codeine, and salicylamide aren't really affected by this pathway. Still, since adrafinil is unregulated and hard on the liver, please run it by your doctor or pharmacist before mixing the two.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 3

Acetaminophen, Caffeine, Codeine, Salicylamide also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, adrafanil might reduce levels and clinical effects of CYP3A4 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.

Here's the thing to know about this combo pill (which blends acetaminophen, caffeine, codeine, and salicylamide). Adrafinil turns into modafinil in your liver, and modafinil can mildly speed up a liver enzyme called CYP3A4. Faster CYP3A4 can clear some of these ingredients a bit quicker, which in theory could slightly weaken their effect. Keep in mind this is a theoretical concern based on how the drugs work, not strong real-world data.

One nuance: codeine is a prodrug that your body converts (mainly through a different enzyme, CYP2D6) into morphine for pain relief, so the CYP3A4 angle is a minor player here. Bottom line, this isn't an alarming pairing, but please run it by your pharmacist or doctor before combining them.

STIMULANT DRUGS

3 of 3

Acetaminophen, Caffeine, Codeine, Salicylamide also falls under STIMULANT DRUGS.

Theoretically, taking adrafinil with stimulant drugs might have additive stimulant effects.
Adrafinil is a precursor to the prescription drug modafinil, which has stimulant effects.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

This combination pill has several ingredients, but the one that matters here is the caffeine. Caffeine is a stimulant, and adrafinil turns into modafinil in your body, which is also a wake-up type stimulant. Stacking the two could add up, so you might feel jittery, anxious, have a racing or pounding heart, trouble sleeping, or higher blood pressure.

To be fair, this is a theoretical concern based on how the drugs work, not strong proof, and the caffeine dose in these tablets is usually modest. Still, adrafinil is unregulated and can stress the liver, and your tablet already contains acetaminophen, which is also processed by the liver. Please check with your pharmacist or doctor before combining them.
