Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP1A2 activity in vitro.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for your Combogesic (acetaminophen plus ibuprofen). Adrafinil turns into modafinil in your body, and modafinil may slightly speed up a liver enzyme called CYP1A2. Since acetaminophen is partly handled by this enzyme, the theory is that your body might clear it a little faster, which could make it work a touch less well for pain or fever.
I want to be honest: this is a theoretical concern based only on lab data, not real-world reports, so I wouldn't lose sleep over it. The bigger worry with adrafinil is its real strain on the liver, and acetaminophen is also processed by the liver. Before pairing these, please check in with your pharmacist or doctor.

Here's the quick picture on combining adrafinil with Combogesic (the acetaminophen and ibuprofen combo). Once your liver turns adrafinil into modafinil, that modafinil may slightly slow down a liver enzyme called CYP2C9. Ibuprofen is partly cleared by that same enzyme, so in theory it could hang around a bit longer and build up. That could mean a higher chance of ibuprofen's usual side effects, like stomach upset, heartburn, or, with heavier or longer use, stomach irritation or kidney strain. The acetaminophen part isn't really handled by CYP2C9, so it's the ibuprofen side we'd watch.
This is a theoretical concern based on lab data, not real-world reports, so for occasional use it's likely minor. Still, it's worth a quick chat with your pharmacist or doctor, especially since adrafinil itself is unregulated and hard on the liver.
