Alprazolam falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Adrafinil interacts with.

Theoretically, adrafanil might reduce levels and clinical effects of CYP3A4 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.

Here's the gist with Xanax (alprazolam) and adrafinil. Your body turns adrafinil into modafinil, and modafinil can gently speed up a liver enzyme called CYP3A4. That same enzyme is one of the main ways your body clears alprazolam. So if the enzyme works a little faster, your alprazolam may get broken down a bit quicker than usual.

In plain terms, that could make your Xanax feel weaker or wear off sooner, so anxiety or panic symptoms might not be as well controlled. I want to be honest though: this is a theoretical concern based on how the drugs behave, not from strong studies, and the enzyme effect is described as weak. Still, please loop in your doctor or pharmacist before combining them, especially since adrafinil itself carries liver risks.

What Is Alprazolam Used For?

Brand names: Xanax, Niravam

Alprazolam treats generalized anxiety disorder (GAD) and panic disorder, with or without agoraphobia, in adults. These uses are supported across multiple alprazolam products, including tablets, orally disintegrating tablets, oral solution, and extended-release tablets.

Xanax (tablet) is indicated for the acute treatment of GAD and for panic disorder. Xanax XR (extended-release tablet) and alprazolam extended-release tablets are indicated specifically for panic disorder with or without agoraphobia — not for GAD. Alprazolam orally disintegrating tablets and alprazolam oral solution are indicated for both GAD and panic disorder.
